Once installed visit http://m.ivr.com for a list of service providers for your consideration. Once you have signed up with a service provider they will provide you with your personal credentials to configure Talking SIP Mobile under Settings|Talking SIP Mobile. Now, whenever you need to make a long distance call simply run Talking SIP Mobile (place it in your application dock beside the iPhone's Phone application for easier access) enter in your desired destination from the keypad, or select a destination from your Contacts, and then click the ‘Trigger’ button to initiate a request.
